Service Descriptions and Pricing
Monthly Access
Each month Reliable RN will focus on one of four main categories of overall wellness; Nutrition, Sleep, Activity, Relaxation. Full access involves at a minimum biweekly studies, suggestions and posts that are designed to support your personal health journey. It will also give you access to once a week, 30 min, “open” session that will act as a Q&A period as well as a time to share with others what is working and what continues to be a challenge for you. These sessions will be group sessions where we will benefit from the wisdom of others. As an added bonus if you have a simple question that just “pops” into your head you will be able to email Reliable RN and get a response within 24 hours (the response might be that the question is to complicated/broad to do via email and a on-time session might be needed).
Month to month – $40/month
3-month commitment – $35/month
6-month commitment – $30/month
Nurse Coaching
Work one on one with Reliable RN to focus on a specific health issue that you or your doctor are concerned about (pre-diabetes, weight/BMI management, diabetes control, cholesterol management, stress reduction, sleep, etc. ). Coaching would include monthly access to the website as well as a 1 hour zoom or in person session every 2 weeks focused on developing measurable weekly goals and creating lifelong health habits. This service may be HSA eligible. Weekly sessions available, normal rate is $60.00/session depending on how long the commitment is. On average, a new behavior takes up to 6 weeks to become a habit.
Month to month – $120/month
3-month commitment – $100/month
6-month commitment – $85/month
Professional Advocacy
Reliable RN can accompany you to any medical appointment*, take notes, interact with your care team, set up medications and/or assist with care transitions to make them as safe and stress free as possible. *In person advocacy is dependent on location. Phone or zoom is an alternative for those outside of Reliable RN’s service range.
Hourly rate – $100/hour, 1 hour minimum
Care Transition Consultation
Reliable RN will connect with all those involved in the planning of a care transition. A care transition is when someone moves from one level of care to another for example going from a hospital to a rehab, hospital to home, rehab to home or home to assisted living. The first week of any transition of care is especially challenging. Reliable RN can help you better understand the process and suggest ways to minimize any safety and/or health related concerns during that first week of transition. The one-time fee is for the consult only. Reliable RN is happy to assist during that first week of transition and hourly rates can be negotiated based on your individual needs.
Consult – $125
Care Management (Geriatric and/or Complex Diagnoses)
Reliable RN will work with you, family members and all care team partners to create an environment where you can stay healthy at home despite having challenging or complex medical needs. This could include in home care, home therapists, lab testing, screening appointments, vaccination recommendations, medical appointments and more. Care coordination often involves a number of phone interactions to different members of the care team. The goal of care coordination is to streamline the number of appointments and phone interactions while still allowing you to get the quality of care you desire. All appointments or needed meetings with caregivers and/or medical professionals will be billed at the rate of $100.00/hour in addition to the monthly cost.
Monthly Cost – Varies and is dependent on the complexity of the care coordination needed. The monthly cost covers texts, phone calls and short “check-ins” as needed. Reliable RN is especially skilled at identifying problems early and working with you and your care team to minimize the need for urgent appointments or a trip to the Emergency room. Monthly cost starts at $299.
Support During an Acute Hospitalization
Reliable RN will do a video or phone call 1-2 times per day during an acute hospitalization. The purpose of this support is to answer any questions or concerns you have about the plan of care or test results and to help reduce your overall stress level. The goal is to increase your ability to be your best advocate so you can work more effectively with the care team. It’s critical that you be empowered to ask questions, voice concerns and be heard during a hospital stay. It’s also critical that you be able to listen and fully understand the proposed plan of care which can be very hard when you don’t feel well or are overly stressed.
Acute Hospitalization – $60/day
In Home Safety Checks* (for those living within a 20-mile radius of Falmouth MA)
This is a local program with the goal of identifying health and safety concerns prior to an acute incident (fall with injury, accidental fire, failure to thrive due to lack of adequate food in the home). Reliable RN will make a 20-30 min visit to your home or the home of a loved one two times a month. Each visit will assess cognition, IADL care and home safety. If concerns exist about your overall health and safety during one of these visits, we will together call either your care team or your health care proxy/family member to alert them of concerns.

On Demand Services
Pre medical appointment consult – One time, 15-20 min call/zoom to assist you in creating a list of important questions/concerns that you wish to be addressed at an upcoming appointment.
Post medical appointment consult – One time 15-20 min call/zoom to explain, clarify or educate about any information received from a recent medical appointment.
Test Result or Imaging consult – One time 15-20 min call/zoom to answer any questions you have about recent testing. The role of Reliable RN is not to give medical advice on the testing but to educate on why the test was done, clarify or educate about the result and, if needed, help you formulate follow up questions for your care team.
